2009-2010 school year
      Thank you for submitting your application. Next you need to submit the Additional Materials outlined below. Be sure to read the instructions carefully and then return them to the below address. Thank you and good luck.

ADDITIONAL MATERIALS
    1. You must provide the following documents:
      a. Official transcript from your high school, verifying your GPA, class rank, and test scores.
      b. ACT or SAT Score(s)
      c. For Sophomore & Higher college students, a copy of your college transcript.
      d. A letter of reference from a current HS or college instructor.
    2. Print the signature form* and sign it. Be sure if you are a minor to have your parents sign it, also.
